Thank you for your interest in the LIFT Initiative (Leveraging Innovation for Facilitated Telehealth), administered by the Utah Education and Telehealth Network (UETN) under Utah's Rural Health Transformation Program (RHTP). The deadline to apply is July 31st, 2026.

Before You Begin

Please review the eligibility and compliance requirements below before starting your application. Applicants unable to meet these requirements should not proceed, as applications that do not satisfy them cannot be considered for funding.

By applying, your organization affirms and acknowledges that:

• It is not debarred, suspended, or otherwise excluded from participation in federal programs.
• It will comply with all applicable federal and state laws, regulations, and program requirements, including 2 CFR Part 200, the RHTP Notice of Funding Opportunity (NOFO), the Notice of Award, and RHTP Program Directives.
• Funds will be used only for the purposes and activities approved in this application and any resulting sub-grant agreement.
• It will cooperate with all monitoring, reporting, and evaluation requirements, including quarterly and annual reports and site visits conducted by UETN, DHHS, and/or CMS.
• Continued funding is contingent on satisfactory performance, timely reporting, and the availability of CMS funds.
• If selected, it will acknowledge federal funding support in all public-facing materials, as required by the Stevens Amendment.
• UETN must submit final drafts of this RFA and resulting agreements to DHHS for review prior to release, and DHHS and CMS retain final approval authority over all activities funded through this program.
